I have added input boxes to the "Application for Certification of a Road Course" in a PDF file. You can use this PDF file to complete and print to paper or to a new PDF file.

You can download this PDF file from Road Course Application

If you use this and find any errors, please let me know so that I can correct.

Below is screen print of top portion of first page completed using this input form.

Tom,
In respect to #6...I actually normally leave this blank unless the application is replacing a previously measured course.

However, a "no" was entered in the example in the Course Measurement manual.

And, by entering the "no", the certifier would know that the measurer hadn't just skipped over this portion of the form.

I have updated my "example" so as to not leave a "bad" example.
Ken,

That was a nice idea, modifying the application forms by adding form fields. I've just followed up on this idea by modifying the version currently posted at http://www.usatf.org/events/co...anual/appendix-f.asp by "typewriter enabling" the entire packet of app forms. This means that you can "type" anywhere on any of the forms in the packet using the Typewriter tool in Adobe Reader 7.0 or greater, and you can save the results using just Adobe Reader (as opposed to full Acrobat). The versions you've currently produced have form fields (which my typewriter-enabled versions don't) but if you have only Adobe Reader (rather than full Acrobat or similar software), the filled-out forms can only be printed--they can't be saved electronically. I noticed also that your RoadCourseApplication.pdf is a huge file--nearly three quarters of a megabyte. Do you have any idea why it's so big?

We can combine these ideas by producing versions with form fields that allow saving. But, for now, my typewriter enabled versions provide most of the functionality.

Hi Bob,

Thanks for following up on this. I think your option will work better for most since the users will be able to save the file and then edit later if needed. With my form method,in order to create a electronic version the user would need to print to a PDF file. But, wouldn't be able to edit the file.

However, I think yours would be more usefull if each application was a separate file. And, of course, having a pdf with input boxes that can be saved would be great.

I did look at the "huge" file issue on the Road Course Application PDF.. The application I use, NitroPDF, has a "optimize" option. Using this option,I was able to reduce the size from 746 KB to 61KB.

The two files that I have added form input fields to are located at :

Road Course Application with Input Forms

Bike Calibration Sheet with Input Forms

Once again thanks for updating the applications forms pdf so that they can be completed electronically.
Jim,

In response to your quesitons....

...1. Can I dump the form data into a file? Would be great for cert reviews.

There most likely is some program that can do that but I am not sure on this.

....2. Can calculation formulas be incorporated in input boxes, as one would do in a spreadsheet?

The product I am using, Nitro PDF Professional, does provide option to incorporate calculations. I haven't try to use this feature.

However, I use an Excel spreadsheet to generate and print my "Bicycle Calibration Sheet" worksheet and my "Course Measurement Data Sheet", so just didn't need these calculations myself.

Below are links to download my spreadsheet template.

Measurement Spreadsheet (xls file)

Measurement Spreadsheet in zip file

This template was designed for one rider, doing one pre-calibration, the two course rides, and then one post-calibration.
For each measurement, you may need to make changes as needed for specific situations.

As a certifier, you might be able to use this spreadsheet in your review process.

If you have questions, please let me know. Send email to Ken@KenHardwick.com

Thanks for this. I plan on using it myself as well as posting it on the NY Measurers google group. Two questions:
1. Can I dump the form data into a file? Would be great for cert reviews.
2. Can calculation formulas be incorporated in input boxes, as one would do in a spreadsheet?

Thanks again.

Excel 2003 worksheet and Excel 2007 worksheet

I like them, as they can be saved as PDF for mailng/emailing, or the measurers in my states can email the Excel to me, whichever they prefer.

Duane, Great form for 07. The counts for a 5K come up just short of 5000 meters (.9207...) Is this a function of "rounding"? I tried adding more decimal points but it's still coming up short. I guess it doesn't matter because my adjustment will be the difference from 5000.
Duane after seeing this post i check the excel 2007 worksheet and I am wondering if it is an example or like Nathan says if you uses meters you are short or if you are using miles you are long. I wonder why that works out that way. I have started using meters and using your figures I would be adding 2 7/8" somewhere to make it come out to 5000 meters. If I am wrong could you please explain why you didn't add any distance to the course? Thank you.
Yes, rounding is the issue. That could be an argument for using metric calibration courses, but I didn't make my calibration course metric.

But, when I change the length to 300 meters, it still is not exactly 5000 meters. Using even click figures causes rounding to a partial click for the average. This causes the final length to be slightly off. Knowing the distance it is off, though, will allow you to add the appropriate length to make it a proper course length.

Nice work on this, gentlemen. I will give it a try. FYI, you can save a PDF form electronically by using the free utility "Primo PDF". Once installed, click "Print", select "Primo PDF" from the printer options, and create a new PDF will all your typed-in data.

I realize that this is an old topic but I'm trying to figure out to fill out the "Fillable" application forms available on the website. The links in this topic are all dead end "404 Error". The example in the attached screenshot shows where I run into trouble. Single line forms boxs work but a multiple line box will not allow me to enter calibration data. I guess I missed something in training. I appreciate it if anyone can point me in the right direction.   Thanks

Jim's attachment above works for me.

It is also available on the USATF website at www.usatf.org. They recently revamped their website so any links from previous threads will no longer work. Once you are on the main usatf page go to Resources and then select Course Certification. Then select Certification Procedures, and then click on the banner that says "Application For Course Certification"

The direct link to the above is

https://www.usatf.org/getattac...lable.pdf?lang=en-US

We no longer have direct control of any of our pages on the USATF website.

We do have control of the RRTC website, and plan to keep this site updated with the latest info. Right now though, some of the links on rrtc.net point back to the old USATF website, so are no longer available. This will be updated soon.
