In response to your quesitons....

...1. Can I dump the form data into a file? Would be great for cert reviews.

There most likely is some program that can do that but I am not sure on this.

....2. Can calculation formulas be incorporated in input boxes, as one would do in a spreadsheet?

The product I am using, Nitro PDF Professional, does provide option to incorporate calculations. I haven't try to use this feature.


However, I use an Excel spreadsheet to generate and print my "Bicycle Calibration Sheet" worksheet and my "Course Measurement Data Sheet", so just didn't need these calculations myself.

This template was designed for one rider, doing one pre-calibration, the two course rides, and then one post-calibration.
For each measurement, you may need to make changes as needed for specific situations.

As a certifier, you might be able to use this spreadsheet in your review process.
