It is my suggestion that all State Certifiers include the measurer's e-mail address on the certificate. I realize that there is no specific line for this information but I believe there is adequate space on the two lines for measurer's information. I am also going to suggest that Bob add a line to the form for this information.

Certifiers should also include their e-mail address under their signature line.

To implement this, it's necessary to modify the application forms so measurers will supply e-mail addresses. I have now done this. Check out the new forms at http://www.usatf.org/events/courses/certification/forms/appforms.pdf

As for the Certificates, it shouldn't be necessary to add a whole new line. It should be enough to change the label for "Measured by" from "name, address, & phone" to "name, address, phone & e-mail". I've made up a sample you can view at http://www.rrtc.net/certificate_sample.pdf - but please note: this sample is only a static pdf, not the dynamic pdf with form fields that you can fill out and print on your computer. I can make up those dynamic forms in both one-signature and two-signature versions. But I'd like to avoid getting a flood of requests from every certifier who already has a customized certificate form. So, for now, I think it's best to use the existing forms and simply add the measurer's e-mail address (when you know it) in the "Measured by" space. I'll supply the new forms as certifiers need new customized forms.

OK, I've added a label requesting "E-mail address" on the Application for Renewal in question 6 (Person Requesting Renewal). Once again, you can get the new form at http://www.usatf.org/events/courses/certification/forms/appforms.pdf

Of course, the person requesting renewal isn't necessarily the measurer. So, while this information may help you communicate with the person who requests renewal, it isn't necessarily transferrable to the Certificate.
